Buckinghamshire Healthcare NHS Trust is seeking a Rapid Response Intermediate Care Community Healthcare Assistant to work with the Aylesbury Rapid Response & Intermediate Care team, delivering personal, social and rehabilitative care in patients' own homes.

You will collaborate with Occupational and Physiotherapists, support timely discharges from hospital, and take responsibility for care delivery, equipment and supervised exercises while you learn toward a Level 3 Health and Social Care

Qualificações

• Full UK driving licence and access to a car for work purposes.
• Willingness to work towards a Level 3 Qualification in Health and Social Care if not already held.

Responsabilidades

• Provide personal and rehabilitative care to adults in patients\' own homes.
• Collaborate with Registered Occupational Therapists and Physiotherapists to deliver care.
• Deliver delegated non-clinical tasks to support team efficiency.
• Support patients to remain safely in their homes and assist with exercises as directed.
