Job summary

Buckinghamshire Healthcare NHS Trust is seeking an experienced Occupational Therapist to join the Rapid Response & Intermediate Care (RRIC) team in Buckinghamshire. The role focuses on expert in-home interventions, enabling recovery and avoiding hospital admissions.

You will support early discharge and care at end of life within a multidisciplinary team. Based in the Aylesbury area, you will collaborate with physiotherapists, nurses, pharmacists and support staff to deliver timely,

Qualifications

• Triage and specialist assessment using OT skills and experience.
• Plan care programmes to promote health gain and maximise independence.
• Actively case manage patients through their journeys.
• Allocate workloads within the Rapid Response & Intermediate Care team.
• Collaborate with MDT professionals to ensure timely care.

Responsibilities

• Triage, comprehensive specialist assessment, review and evaluation of patients and carers using OT expertise.
• Plan programmes of care to promote health gain and maximise independence.
• Allocate appropriate workloads to RRIC team members.
• Work with a range of professionals to ensure care is appropriate and delivered promptly.
• Demonstrate leadership in care planning, coordination and delivery.
