Job summary

Buckinghamshire Healthcare NHS Trust is seeking an Occupational Therapist to join the Rapid Response & Intermediate Care service. The post focuses on triage, assessment and case management to maintain patients at home and support early hospital discharge, including end-of-life care.

You will work within a multi-disciplinary team, coordinating workloads and contributing to clinical pathways. Applicants must have HCPC registration as an occupational therapist and a valid driving licence.

Qualifications

• Professional OT qualification and registration are required.
• Post-graduate training in relevant areas is desirable.
• Ongoing CPD and development are expected.

Responsibilities

• Provide triage, specialist assessment, review, and case management of patients and carers.
• Plan care to maximise independence and health gain, and manage patients’ journeys.
• Deliver expert interventions and rehabilitation to help patients stay at home and support early discharge and end-of-life care.
• Allocate workloads within the Rapid Response & Intermediate Care (RRIC) team.
