Buckinghamshire Healthcare NHS Trust is recruiting an Occupational Therapist for the Aylesbury team within the Rapid Response & Intermediate Care service. You will specialise in assessment of patients and carers, planning care to keep people independent at home, and case managing them through their care journeys.

You will work in a multi-disciplinary community team responding to crisis, supporting early hospital discharge and providing end-of-life care.

Qualificações

• HCPC registration is required.
• Post-registration CPD and ongoing professional development.
• Postgraduate clinical training in areas like long-term conditions, respiratory, cardiac, trauma, orthopaedics and end of life.

Responsabilidades

• Triage and comprehensive specialist assessment of patient and carer needs.
• Plan care programmes to promote health and independence.
• Actively case manage patients through their care journeys.
• Allocate workloads to RRIC team members.
• Collaborate with multi-disciplinary professionals to ensure timely, appropriate care.
• Demonstrate leadership in care planning and coordination.
• Support development of the clinical pathway.
