Qualified Social Worker, Adult Community Team

Pay rate: £30 per hour

Vitalis is recruiting an experienced Social Worker to work in an Adult Community team in Salford.

An exciting opportunity has arisen for an experienced Social Work Practitioner to join Salford's Adult Health and Social Care Contact Team.

The Adult Health and Social Care Contact Team is the first point of contact for residents of Salford who may need access to health or social care services. In this role, you will work closely with colleagues to provide information, advice, and support to adults and their carers who may be eligible for assessment and care services under the Care Act 2014.

Our goal is to support people in living well and safely at home by conducting strength-based conversations to promote wellbeing. We collaborate with other organizations to ensure individuals are connected with their local communities and can work towards achieving their personal outcomes.

The Adult Social Care workforce aims to improve outcomes for residents of Salford by developing a skilled and motivated team. We invest in continuous professional development and maintain high standards in social work practice. Candidates should hold a degree in social work, be registered with Social Work England, and have evidence of ongoing professional development, with a strong understanding of the Care Act, Mental Capacity Act, and Safeguarding Adults.
