Job summary

ADAPT Community Network seeks an Entitlements Coordinator to manage public benefits for people supported by ADAPT and affiliates. You will coordinate applications, renewals, and documentation, ensuring compliance with federal, state, and local regulations while collaborating across Residential Directors, Program Leadership, Finance, Admissions, and Care Management.

You will maintain confidential entitlement records, communicate with government agencies, and support timely determinations.

Qualifications

• Bachelor's degree required.
• Experience administering or coordinating public benefits preferred.
• Knowledge of Medicaid, SNAP, Social Security helpful.

Responsibilities

• Coordinate applications, renewals, and documentation for public benefits.
• Verify benefit eligibility for new admissions and ongoing renewals.
• Prepare and submit documentation to Federal/State/local agencies.
• Monitor and follow up on benefit determinations and notices.
• Maintain confidential entitlement records and ensure HIPAA compliance.
• Collaborate with Residential Directors, Program Leadership, Finance, Admissions, and Care Management.
