ADAPT Community Network seeks a dedicated Case Manager to develop, implement, and monitor Life Plans for individuals in residential settings. You will ensure services align with regulations, advocate for rights, and coordinate with families and external partners to promote dignity and quality of life.

The role involves advocacy, data collection, and collaboration with residential and clinical staff to deliver person-centered, coordinated care. Full-time position with opportunities for growth.

Qualifications

• Bachelor’s Degree in Human Services or related field is required.
• Minimum of one year of experience with individuals with developmental disabilities.
• Ability to read, write, speak, and understand English.
• Strong interpersonal and communication skills with diverse stakeholders.
• Excellent organizational and time management skills.
• Proficiency in Microsoft Office and electronic record systems.
• Ability to maintain confidentiality and exercise discretion.
• Ability to work independently or in a team with flexible scheduling.
• Ability to work flexible hours.

Responsibilities

• Develops, monitors, and updates Individualized Service Plans.
• Coordinates with residential, clinical staff, and providers to ensure coordinated support.
• Documents progress notes and family contacts.
• Advocates for rights and ensures benefits are applied.
• Maintains regular contact with families and guardians.
• Leads interdisciplinary team meetings and follows up on recommendations.
• Assists with Life Plan implementation and data collection.
