Job summary

Ashfield District Council is seeking an Assertive Outreach Officer to join the Housing Options Team, working with households who are homeless or facing urgent housing need.

You will conduct detailed needs assessments, develop tailored action plans in partnership with service users and agencies, and support individuals to secure settled accommodation and sustain tenancy responsibilities.

Qualifications

• Carrying out detailed needs assessments and developing tailored action plans.
• Building trust with service users and supporting them to improve housing circumstances.
• Collaborating with social care, health services and voluntary sector to identify needs and solutions.
• Maintaining accurate case records and contributing to service improvement.

Responsibilities

• Identify support needs and tackle barriers to housing for households who are homeless or facing urgent housing need.
• Help individuals and families move on into settled accommodation and understand tenancy responsibilities.
• Develop and implement action plans in partnership with service users and partner agencies.
• Work with landlords, support providers and internal teams to find creative housing solutions and monitor outcomes.
