Job summary

Leeds Community Healthcare NHS Trust is seeking a motivated practitioner to join the new Proactive Support Service, focusing on children and young people with complex needs. You will work in a multidisciplinary team to provide assessment, direct intervention and system-wide support to families and carers.

Qualifications

• Registered Nurses with experience in Learning Disabilities, Mental Health, Children's Services or Complex Care.
• Qualified social workers or other registered practitioners with relevant experience will be considered.
• Experience in trauma-informed and relational approaches is desirable.

Responsibilities

• Provide assessment, consultation, direct intervention and system-wide support.
• Collaborate with families, carers, residential providers, social care, education and health colleagues.
• Support CNOD pathway and prevent placement breakdown.
