Job summary

Central London Community Healthcare NHS Trust is recruiting a Neighbourhood Case Coordinator to support proactive care in Hertsmere. The role combines clinical assessment, care coordination and administration to help residents stay well and avoid unnecessary hospital admissions.

You will visit patients at home or in clinics, conduct holistic assessments, produce clinic letters and MDT minutes, and liaise with health and social care professionals to ensure well-coordinated care.

Qualifications

• Experience in health/community settings and ability to support care coordination.
• Evidence of strong administrative skills and ability to produce clinic letters and MDT minutes.
• Full UK driving licence with access to a car for business use.

Responsibilities

• Visit patients at home or in clinics to assess/monitor long-term conditions.
• Provide advice and support to enable patients to live well and avoid hospital admissions.
• Liaise with health and social care professionals to ensure coordinated care.
• Produce clinic letters, MDT meeting minutes, and manage appointment bookings.
