Job summary

Hull and East Yorkshire Hospitals NHS Trust is seeking a Care Co-ordinator to support the practice in delivering excellent patient care by coordinating elements of care from multi-disciplinary teams. You will navigate services, coordinate MDT meetings, and act as a single point of contact for health professionals and patients within your remit.

• Clear communication with multidisciplinary teams
• Ability to co-ordinate care pathways
• Administrative ability to manage records and reporting

Responsibilities

• Co-ordinate the work of healthcare professionals and non-clinical staff involved in patient care
• Coordinate MDT meetings and prepare information for review actions
• Act as single point of contact for health professionals and patients within remit
• Support GPs with administrative aspects of QOF
• Maintain accurate records and produce performance data
