We seek a Through the Gate Coordinator to support people transitioning from custody to the community, meeting individuals in prison to plan tailored support with their Support Worker and continuing guidance post-release.

You will work from prisons, community hubs and probation offices, collaborating with pre-release teams, probation services and internal departments to provide holistic, person-centred

Qualifications

• Relationship building with Probation stakeholders and Service Users in a way that's genuine and non-judgmental.
• Safeguarding awareness to identify risks and protect clients.
• Meticulous IT and administrative record-keeping to measure client progress.
• Strong organisation to manage a busy caseload and maintain detailed records.
• Resilience to work in challenging conditions while remaining committed.
• Flexibility to adapt as referral flows change frequently.

Responsibilities

• Community-based support: guide individuals per action plans, aiding housing, employment, mental health and social connections.
• Collaboration: work with internal and external colleagues as part of a wider support package.
• Caseload management: handle multiple individuals while balancing needs and admin duties.
• Quality Recording: maintain thorough, up-to-date records on IT systems, ensuring progress is tracked.
