Job summary

Practice Plus Group is seeking a Primary Care Nurse to join the Primary Care team at HMP Drake Hall, a women’s closed prison near Eccleshall in Staffordshire, with about 340 inmates. You will deliver nurse-led care within a multidisciplinary team and help prevent hospital admissions.

This full-time role offers 37.5 hours per week (Mon–Fri 7.15am–5.15pm, every other weekend), a rolling rota, and a salary between £34,428 and £41,659 per year, with induction and development opportunities.

Qualifications

  • Registered nurse with Adult scope (NMC).
  • Evidence of Continued Professional Development (CPD).
  • Understanding of multi-disciplinary and multi-agency working.
  • Non-judgemental and compassionate approach.
  • Excellent communication skills across clinical and non-clinical settings.

Responsibilities

  • Deliver nurse-led services within a multi-disciplinary team.
  • Manage medication rounds and dispense controlled medications.
  • Attend emergency response situations and undertake reception screening.
  • Lead nurse-led clinics and provide evidence-based care.
  • Forge links with partners to support patient care and safety.

Hours
  • 37.5 hours a week working Monday - Friday 7.15am - 5.15pm
  • Every other weekend, 7.30am - 5.30pm
  • 4 week rolling rota
Salary

This position is full time, you’ll receive an annual salary between £34,428 - £41,659 per annum - depending on experience.

What you'll be doing

As a Primary Care Nurse your responsibilities will vary. You will

  • Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
  • Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
  • Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
  • Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
  • Deliver non-judgemental care that makes a real difference
  • Inspire excellence by forging strong links with our partners and stakeholders
  • Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
  • Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.
