Job summary

Practice Plus Group is recruiting a Senior Primary Care Nurse to join our Primary Care team at HMP Stocken, a category C Prison in Rutland. You will provide high quality healthcare to our patients, within the healthcare department with some prison wing work required.

This role leads a multi-disciplinary team, conducts medication rounds, assesses patients, and delivers evidence-based, patient-centred care. We offer bespoke induction, a competency framework, regional and national development

Qualifications

  • Registered nurse – Adult with NMC registration.
  • Evidence of Continued Professional Development.
  • Understanding of the complexities of multi-disciplinary and multi-agency working.
  • Excellent communication skills.
  • Proactive problem solving skills.
  • Motivated as an individual and when working in a team.

Responsibilities

  • Lead a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions.
  • Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening.
  • Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
  • Deliver non-judgemental care that makes a real difference.
  • Inspire excellence by forging strong links with our partners and stakeholders and providing a service that people can trust, feel safe within, and feel proud of.
  • Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
  • Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams and supporting each other to do a great job.

We have an exciting opportunity for a Senior Primary Care Nurse to join our Primary Care team at HMP Stocken, a category C Prison located in Rutland. As a Senior Primary Care Nurse you will be working within the primary care team, you will be based within the healthcare department and some prison wing work required.

  • Monday - Sunday service
  • 37.5 hours per week
  • Weekdays: 7:30am - 5.30pm or 7:30am - 6.30pm
  • Weekends and Bank Holidays 8:30am - 5.30pm
  • 2 weekends out of 4
Salary

This position is full time, you’ll receive an annual salary between £43,096 - £49,175 per annum FTE - depending on experience.
