Job summary

Practice Plus Group’s Primary Care team at HMP Brinsford in Wolverhampton is seeking a Primary Care Nurse to deliver nurse-led services within the healthcare department and on prison wings.

You will work in a multidisciplinary team, perform medication rounds, run clinics, and contribute to learning and development opportunities while providing compassionate care in a secure environment.

Responsibilities

  • Deliver nurse-led services within a multi-disciplinary team to prevent hospital admissions
  • Undertake medication rounds and dispense controlled medications
  • Attend emergency responses and perform patient assessments
  • Lead nurse-led clinics delivering patient-centred care
  • Provide compassionate, non-judgemental care and engage with feedback to improve services
  • Forge links with partners and stakeholders to promote best practice

Hours:
  • Monday - Sunday service
  • 37.5 hours per week
  • Long-days - 7am - 7.30pm
  • Nights - 7pm - 7.30am
Salary:

This position is full time, you’ll receive an annual salary between £34,428 - £41,659 per annum - depending on experience.

What you'll be doing

As a Primary Care Nurse your responsibilities will vary. You will

  • Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
  • Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
  • Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
  • Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
  • Deliver non‑judgemental care that makes a real difference
  • Inspire excellence by forging strong links with our partners and stakeholders
  • Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
  • Share knowledge, skills and expertise to ensure safe care, building strong multi‑disciplinary teams.
