Practice Plus Group is seeking a Primary Care Nurse to join the Primary Care team at HMP Full Sutton, Full Sutton, York. This rural site offers parking, an on-site gym, café, and team-building events, with structured development opportunities for staff in a supportive hospital-prison environment.

You will work within a multi-disciplinary team to deliver nurse-led services, manage medicines, and support patient care through clinics and rapid response scenarios, continuing professional development

Qualifications

  • Registered nurse – Adult in good standing with NMC.
  • Current professional qualification (NMC) and evidence of CPD.
  • Experience in multidisciplinary settings and working with diverse teams.
  • Non-judgmental and compassionate approach to care.
  • Excellent communication and patient-facing skills.

Responsibilities

  • Deliver nurse-led services within a multi-disciplinary team to prevent hospital admissions.
  • Undertake medication rounds and dispense controlled medication as required.
  • Attend emergency responses and conduct patient assessments and reception screening.
  • Lead nurse-led clinics delivering evidence-based care tailored to patient groups.
  • Provide high-quality, non-judgemental care and continuous quality improvement.
  • Foster strong links with partners and stakeholders to support care pathways.

We have an exciting opportunity for a Primary Care Nurse to join our Primary Care team at HMP Full Sutton, a category A men's Prison located in Full Sutton, York. This site is located in a rural location with their own parking on site, an on-site gym, café, team building days and well-being events annually, regular 1-1's and good scope to develop.

Hours

37.5 hours a week, working pattern is 4 shifts a week, 2x long days 7am-7.15pm and 2x main shifts, 8-4pm, including 1 in 4 weekends, 2 long days. Future potential night rotation.

Salary

This position is full time, you’ll receive an annual salary up to £41,659per annum, depending on experience.

What you'll be doing

As a Primary Care Nurse your responsibilities will vary. You will

  • Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
  • Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
  • Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
  • Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
  • Deliver non-judgemental care that makes a real difference
  • Inspire excellence by forging strong links with our partners and stakeholders
  • Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
  • Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.
