Job summary

Practice Plus Group is recruiting a Primary Care Nurse for HMP Thorn Cross. You will join a multidisciplinary primary care team delivering nurse-led services in a secure environment, aiming to reduce hospital admissions and provide high standards of patient care.

The role requires UK work rights and compliance with security vetting. Working 37.5 hours per week over 4 days, you will handle medication rounds, clinics, and urgent care while collaborating with partners to promote best practice and

  • Registered nurse with adult scope and NMC registration.
  • Evidence of Continued Professional Development is required.
  • Experience understanding multi-disciplinary and multi-agency working is expected.
  • Approach: non-judgemental and compassionate.
  • Excellent communication skills are essential for this role.

Responsibilities

  • Deliver a nurse-led service within a multi-disciplinary team to prevent hospital admissions.
  • Undertake medication rounds, dispense controlled medication, and respond to emergencies.
  • Run nurse-led clinics for your patient group with MDT support.
  • Provide evidence-based care and use patient feedback to personalise care.
  • Foster strong links with partners and stakeholders to drive quality and safety.

HMP Thorn Cross is a Category D / open prison in Appleton Thorn, Warrington, housing around 430 males.

  • Monday - Sunday service
  • 37.5 hours per week over 4 days
  • Weekdays 7.30am - 5.30pm
  • Weekends 07.15am - 12.00pm
  • 1 weekend in 3
Salary

This position is full time, you'll receive an annual salary of between £34,428 - £41,659 per annum depending on experience.

Candidates are required to have a Legal right to work in the UK and due to the nature of the role, offers of employment will be subject to additional vetting and security checks.

What you'll be doing
  • Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
  • Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
  • Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
  • Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
  • Deliver non-judgemental care that makes a real difference
  • Inspire excellence by forging strong links with our partners and stakeholders
  • Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
  • Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.
