Job summary

Practice Plus Group is seeking a Primary Care Nurse to join its Health in Justice team in the UK. You will work in a nurse-led model with a multidisciplinary group, focusing on preventing hospital admissions and delivering clinical care across clinics and ad hoc emergency responses.

The role emphasizes compassionate, evidence-based practice, ongoing professional development, and strong communication with partners to improve patient outcomes.

Qualifications

• Registered nurse with current NMC registration and adult care competence.
• Strong communication and teamwork within multi-disciplinary teams.
• Experience in hospital, community, or similar settings preferred.

Responsibilities

• Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions.
• Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening.
• Provide nurse led clinics and deliver evidence-based care tailored to patient needs.
• Promote best practice and continuous quality improvement within the team.

Job description

As a Primary Care Nurse your responsibilities will vary. You will

• Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions


• Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening


• Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.


• Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.


• Deliver non-judgemental care that makes a real difference


• Inspire excellence by forging strong links with our partners and stakeholders

This position is full time, youll receive an annual salary between £34,428 - £41,659 per annum - depending on experience.
