Job summary

Practice Plus Group is seeking a Primary Care Nurse to join our Primary Care team at HMP Kirkham in Preston. Based in the prison's Healthcare department, you will join a dedicated primary care team of 10 serving a capacity of 700 male inmates and delivering nurse-led clinics and urgent care in a secure setting.

The role is full-time with onsite parking, access to the staff gym, an onsite farm shop and café, plus bespoke training and development opportunities to support your professional growth

Qualifications

  • Registered nurse – Adult (NMC registration).
  • Evidence of Continued Professional Development.
  • Experience with multidisciplinary and multi-agency working.
  • Non-judgemental and compassionate approach.
  • Excellent communication skills.

Responsibilities

  • Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions.
  • Undertake medication rounds and dispense controlled medication.
  • Attend emergency response situations and assess patients.
  • Deliver nurse led clinics for your patient group with support from a multi-disciplinary team.
  • Provide evidence-based care, listen to and act on patient feedback to personalise care.
  • Share knowledge and expertise to ensure safe care and build strong teams.

Hours
  • Monday - Thursday 8am - 7pm
  • Friday - 8am - 4:30pm
  • Weekend - 8am - 12pm
  • 1 weekend in 4
  • Rotas done 2 months in advance
Salary

This position is full time, you’ll receive an annual salary between £34,428 - £41,659 per annum depending on experience

What you’ll be doing

As a Primary Care Nurse your responsibilities will vary. You will

  • Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
  • Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
  • Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
  • Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
  • Deliver non-judgemental care that makes a real difference
  • Inspire excellence by forging strong links with our partners and stakeholders
  • Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
  • Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.
