Job summary

You will be part of a group of PCN clinical pharmacists within NHS. You will develop through team clinical supervision sessions and benefit from direct mentoring with an experienced GP/ Lead Pharmacist.

Access to NHS England's accredited training pathway to practice and prescribe safely in primary care. The role aims to improve patient outcomes by delivering clinical medicines expertise across a multidisciplinary practice team, and to promote safe, cost-effective prescribing aligned with CCG

Qualifications

• Knowledge of NHS policies, processes and structures.
• Understanding of GDPR and Caldicott requirements.
• Ability to use GP systems – EMIS Web is desirable.

Responsibilities

• Patient facing long‑term condition and medication review clinics.
• Review medicine plans and provide recommendations to clinicians.
• Medicines support in person or by telephone; patient queries handling.
• Manage repeat prescribing, electronic prescribing and audits.
• Contribute to medicines safety, risk stratification and cost effectiveness.
