Job summary

The Pharmacist Network is hiring an onsite Clinical Pharmacist to support general practice teams across primary care. You will resolve medication-related queries, conduct reviews, and work with pharmacists and technicians to enhance patient outcomes and access to healthcare.

Requires at least 12 months in general practice, GPhC registration, and an independent prescribing qualification. CMME systems like EMIS/System One experience is preferred and CPPE Primary Care pathway near completion.

Qualifications

• Minimum 12 months experience in general practice/primary care.
• Registered with GPHC (GPhC registration).
• Independent prescribing qualification.
• Experience with EMIS or System One.
• Completed CPPE Primary Care pathway or near completion.
• Able to commute to onsite location or live nearby.

Responsibilities

• Resolve medication-related queries as part of a multidisciplinary team.
• Perform polypharmacy reviews and SMRs.
• Conduct face-to-face medication reviews with patients.
• Utilise skill mix to optimise medicines management.
• Improve patient outcomes and access to healthcare.
• Provide expert medicines advice to clinicians and patients.
• Lead medicines optimisation and quality improvement efforts.
