Job summary

NHS Jobs is seeking a patient‑facing Clinical Pharmacist to support medicine optimisation across primary care settings. You will review medicines, lead clinically focused reviews for long‑term conditions, support care homes and domiciliary patients, and contribute to safety, audits and public health initiatives.

You will work with GPs, nurses and practice teams to improve prescribing, provide medicines information and run clinics, telephone support, and educational sessions for staff.

Responsibilities

• See patients with single or multiple medical problems where medicine optimisation is required.
• Review the ongoing need for each medicine, monitoring needs and support patients with medicines taking to ensure best use.
• Make appropriate recommendations to Senior Pharmacists or GPs for medicine improvement.
• Undertake clinical medication reviews with patients and produce recommendations for prescribing and monitoring.
• Work with care home staff to improve safety of medicines ordering and administration.
• Provide patient facing clinics for those with questions about their medicines in the practice.
• Provide a telephone help line for patients with questions about their medicines.
• Answer medicine-related enquiries from GPs, staff and patients with queries about medicines and provide follow up.
