Job summary

The Confederation, Hillingdon CIC is seeking a Clinical Pharmacist to join a multidisciplinary Primary Care Network team. You will develop and run processes for repeat prescription reauthorisation, medicines management on transfer of care, and safer prescribing.

You will work directly with patients, assessing and treating conditions and supporting long-term care management. You will engage in CPPE training over 18 months, with supervision by a Clinical Pharmacist Manager, and the option to

Qualifications

• Mandatory registration with General Pharmaceutical Council (GPhC).
• Masters degree in pharmacy (MPharm) (or equivalent).
• Minimum of 2 years post-qualification experience.
• Independent prescribing qualification or working towards/intent to gain it.
• Understanding of primary care prescribing concepts and rational prescribing.

Responsibilities

• Undertake clinical medication reviews with patients in practice, care homes or domiciliary visits.
• See patients with multiple conditions requiring medicine optimisation and advise on prescribing.
