Job summary

Liverpool University Hospitals NHS Foundation Trust is seeking a Clinical Pharmacist to join the Primary Care Network, delivering patient-centered pharmaceutical care across member practices. The post holder will support medicines optimisation, improve outcomes, reduce harm and contribute to efficient primary care services.

• GPhC registration and up-to-date professional practice.
• Experience in medicines optimisation and clinical reviews.
• Experience working with multi-disciplinary teams in primary care.

Responsibilities

• Undertake structured medication reviews and medicines optimisation activities.
• Provide prescribing support and clinical advice to clinicians and patients.
• Collaborate with GPs, nurses and other professionals to optimise medicines.
• Support care homes and implement national and local prescribing guidance.
• Reconcile medicines after hospital discharge and monitor high-risk medicines.
