You will run long-term condition clinics, perform structured medication reviews, and collaborate with GP colleagues, nurses and care staff to ensure safe and effective medicines use for diverse patient groups.

Responsibilities

• Patient-facing long-term condition clinics reviewing medicines and monitoring needs, with recommendations to GPs for improvement
• Conduct patient-facing clinical medication reviews and provide prescribing/monitoring recommendations to senior pharmacists, nurses or GPs
• Perform patient-facing care home medication reviews and advise care home staff to improve medicine safety in ordering and administration
• Carry out domiciliary clinical medication reviews and refer patients to multidisciplinary teams as appropriate
• Manage common/minor/self-limiting ailments within scope of practice and refer to GPs or other professionals when needed
• Provide patient-facing and telephone medicines support, answer enquiries, and monitor outcomes of changes
• Review medicines related to unplanned hospital admissions and implement changes to reduce high-risk prescribing
• Reconcile medicines on hospital discharge and ensure continuity of supply to high-risk groups
• Signpost patients to appropriate healthcare professionals for timely care
• Produce and manage repeat prescribing policies and monitoring of reauthorisation processes
• Identify high-risk patient cohorts through targeted reviews and audits
• Support medicines quality improvement, respond to MHRA alerts and ensure NICE/CQC alignment in prescribing
• Educate and train the primary care team on therapeutics and medicines optimisation and support public health campaigns
