Job summary

The Practice Nurse role covers three GP practices—Whitehouse Surgery, Brooklands Health Centre and Neath Hill Health Centre, with Whitehouse as the main base. The post focuses on long-term condition reviews, health promotion, vaccinations and cervical screening, wound care and patient assessment.

Hours can be full-time at 40 a week or part-time by agreement. Fit depends on current NMC registration and substantial experience working as a practice nurse.

Qualifications

• NMC registration and substantial practice nurse experience.
• Experience delivering vaccination and immunisation programmes.
• Knowledge of NICE guidance and safeguarding requirements.
• Ability to maintain accurate clinical records and contribute to QOF.
• Commitment to clinical governance and ongoing professional development.

Responsibilities

• Assess, plan and review care for patients with long-term conditions.
• Provide health education, lifestyle advice, screening and preventive interventions.
• Deliver adult and childhood vaccination programmes and cervical screening.
• Carry out cervical cytology consultations under the national screening programme.
• Give contraception and sexual health advice within your competencies.
• Assess and manage uncomplicated wounds using evidence-based practice.
• Prioritise routine, urgent and emergency patients.
• Maintain safeguarding for children and vulnerable adults.
• Keep accurate records and contribute to audits and improvements.
• Collaborate with the multidisciplinary team to deliver safe, coordinated care.
