Job summary

Chawton House Surgery, a GP practice, is recruiting a practice nurse to its primary care team. The post centers on chronic disease management, immunisations, cervical screening, wound care and health promotion, working alongside the GPs and wider practice staff. Patients are seen over the long term in a general practice setting, with predictable working hours.

Qualifications

• Registered nurse with current NMC registration.

Responsibilities

• Provide general nursing care, assessment, treatment and advice to patients of all ages.
• Support management of patients with long-term conditions (diabetes, asthma, COPD, cardiovascular disease).
• Administer adult and childhood immunisations under guidance and PGDs.
• Undertake cervical screening where trained.
• Wound assessment, dressings, and wound care including suture removal.
• Carry out routine health checks, clinical observations, investigations and preventative care.
• Provide health promotion on lifestyle, diet, exercise, smoking cessation and disease prevention.
• Travel health advice and vaccinations where trained.
• Assist GPs with minor surgical procedures when required.
• Recognise changes in patients' conditions and safeguarding concerns; refer as needed.
• Maintain accurate, confidential patient records and follow infection control policies.
