Job summary

Weald Primary Care Network is seeking a Practice Nurse to support public health initiatives and run nurse-led clinics within the PCN. The role involves immunisations, vaccinations, and managing complex care for housebound and care home residents in partnership with GPs.

You will deliver clinics, provide clinical support and guidance to care home staff, and engage with community and secondary care services to ensure integrated care. Ongoing CPD and adherence to NHS standards are expected.

Responsibilities

• Support public health and screening programmes, including immunisations and vaccinations.
• Deliver expert nursing assessments and manage complex care needs of housebound and care home residents.
• Create trusting partnerships to communicate and explain complex medical issues to patients and carers.
• Mentor, support and supervise new-to-general-practice nurses and student nurses within the PCN.
