Job summary

Living Well Partnership is seeking an experienced Practice Nurse to join our multidisciplinary team of GPs and nurses. You will deliver high-quality nursing care, health assessments, immunisations, and chronic disease management in line with NMC standards and our policies.

You will educate patients on healthy lifestyles, maintain accurate records in the practice system, and contribute to continuous service development through meetings and audits.

Qualifications

• NMC registered and eligible to practice in the UK.
• Experience in chronic respiratory disease management is desirable.
• Strong communication and patient education skills.
• Ability to work as part of a multidisciplinary team.

Responsibilities

• Conduct patient assessments including history taking, examinations and vital signs.
• Administer treatments and procedures such as wound care, ECGs, injections.
• Provide immunisations including travel vaccines and cervical screening.
• Support management of chronic conditions including diabetes, COPD, CVD.
• Maintain accurate patient records in the practice's system.
• Collaborate with GPs and healthcare professionals to optimise care.
• Promote healthy lifestyles and preventive care.
