Responsibilities

• Provide general nursing care, assessment, treatment and advice to patients of all ages.
• Support management of long-term conditions (diabetes, asthma, COPD, cardiovascular disease).
• Undertake cervical screening and immunisations as trained.
• Carry out wound care, health checks and health promotion.
• Maintain confidential patient records and safeguarding.
• Work with GPs and the practice team to deliver patient-centred care.
