About the Role
We are looking for an experienced and dedicated Practice Nurse to join a friendly and established GP surgery.
The successful candidate will work as part of a multidisciplinary team alongside GPs, ANPs, HCAs and other primary care professionals, delivering high-quality, patient-centred nursing care.

Key Responsibilities
- Conducting long-term condition reviews and management
- Managing patients with diabetes, asthma, COPD, hypertension and cardiovascular disease
- Carrying out routine health checks and clinical assessments
- Performing blood pressure checks, ECGs and phlebotomy
- Delivering immunisations and vaccinations
- Providing cervical screening where appropriately trained
- Supporting NHS Health Checks and preventative healthcare
- Providing wound care and dressing changes
- Undertaking medication reviews within scope of practice
- Providing health promotion and lifestyle advice
- Supporting smoking cessation and healthy lifestyle initiatives
- Conducting clinical assessments and developing appropriate care plans
- Recognising and escalating changes in a patient's condition
- Working closely with GPs, ANPs, HCAs and the wider multidisciplinary team
- Maintaining accurate and up-to-date patient records
- Supporting clinical audits and quality improvement
- Ensuring compliance with infection prevention, safeguarding and clinical governance requirements
- Providing a high standard of compassionate and patient-centred care
About You
The ideal candidate will be a confident and experienced Practice Nurse with excellent clinical and communication skills.
You will ideally have:
- Current NMC registration and valid NMC PIN
- Previous Practice Nurse or primary care experience
- Strong knowledge of long-term condition management
- Experience working autonomously within a GP surgery
- Excellent clinical assessment skills
- Strong communication and interpersonal skills
- A professional and compassionate approach
- Good organisational and time-management skills
- Ability to work independently and as part of a multidisciplinary team
- Good IT and clinical record-keeping skills
- Commitment to delivering high-quality patient care
Qualifications & Experience
Essential:
- Registered Nurse qualification
- Current NMC registration
- Valid NMC PIN
- Relevant clinical experience
Desirable:
- Practice Nurse/Primary Care qualification
- Experience in chronic disease management
- Cervical screening qualification
- Immunisation and vaccination training
- Diabetes management experience
- Asthma/COPD experience
- ECG and phlebotomy experience
- Experience with EMIS or SystmOne
- Knowledge of QOF
