Job summary

Alton Street Surgery is seeking a Practice Nurse to join its nursing team, focusing on evidence-based care, respiratory and cardiovascular reviews, chronic disease management, immunisation and screening. The role is largely autonomous with extension coverage on Wednesday evenings until 8.00pm; registration as a nurse and primary care experience are required.

You will support the nursing team, contribute to public health initiatives, and maintain accurate clinical records in SNOMED CT, while

Qualifications

• Registered nurse with current NMC registration.
• Experience in primary care or practice nursing.
• Experience in chronic disease management.
• Infection prevention and control experience.

Responsibilities

• Deliver evidence-based nursing care to the registered patient population.
• Carry out respiratory and cardiovascular disease reviews.
• Manage chronic diseases and quality improvement work.
• Lead immunisation, vaccination and cervical screening programmes.
• Coordinate the nursing team rota and support new-to-practice nurses.
• Support IPC lead and response to clinical emergencies.
• Participate in clinical audits and development of practice protocols.
