Job summary

Alton Street Surgery is seeking a Practice Nurse to join its nursing team, focusing on evidence-based care, respiratory and cardiovascular reviews, chronic disease management, immunisation and screening. The role is largely autonomous with extension coverage on Wednesday evenings until 8.00pm; registration as a nurse and primary care experience are required.

Responsibilities

• Deliver evidence-based nursing care to the registered patient population.

• Coordinate the nursing team rota and support new-to-practice nurses.
• Support IPC lead and response to clinical emergencies.

Job description

Alton Street Surgery, a GP training practice, needs a Practice Nurse to work within its nursing team with HCA support. The role centres on evidence-based nursing care, with a particular focus on respiratory and cardiovascular disease reviews, chronic disease management and health promotion, and includes immunisation and screening work. The post is largely autonomous and includes covering extended access appointments on Wednesday evenings until 8.00pm. Registration as a nurse and primary care experience are needed to be considered.

What the job involves

• Deliver evidence-based nursing care within scope of practice to the registered patient population
• Carry out respiratory and cardiovascular disease reviews, with a particular focus
• Manage chronic diseases and long-term conditions, including quality improvement and assurance work
• Lead on public health campaigns such as flu clinics
• Deliver immunisation, vaccination and cervical screening programmes, with opportunistic health promotion
• Develop and embed a care management programme for frail and housebound patients, and population health approaches targeting inequalities
• Consult with patients and families to assess need, and teach them to manage their conditions
• Provide routine nursing care and wound care in line with NICE guidance and protocols
• Maintain clinical records using SNOMED CT, chronic disease registers and call/recall systems
• Coordinate the nursing team rota and support new-to-practice nurses
• Support the IPC lead, safeguarding and the response to clinical emergencies
• Take part in clinical audits, complaints handling and the development of practice protocols
