Job summary

Cherry Hinton & Brookfields Medical Centres are seeking a Practice Nurse to manage chronic diseases, provide vaccinations, and support cervical screening and wound care. The role includes leading the nursing team and contributing to QOF and audit activities.

You will work with the clinical lead and lead nurse to maintain high standards of patient care, records, and infection control, while delivering evidence-based care to patients with asthma, COPD, and diabetes.

Qualifications

• Post-registration experience in primary care.
• Experience of chronic disease management, respiratory and/or diabetes.
• NMC registration, current and valid.

Responsibilities

• Deliver evidence-based care and health promotion plans for patients with long-term conditions.
• Manage and treat long-term conditions in line with national and local policy.
• Review medication for effectiveness and support adherence to treatment.
• Respond to complex, urgent or emergency presentations, including starting emergency care.
• Lead the nursing team on quality governance and assess competence.
• Maintain accurate patient records and work within safeguarding and infection control policy.
