Job summary

IntraHealth Ltd in the United Kingdom is recruiting a Practice Nurse to join our team at the William Brown Centre. You will deliver basic practice nursing services, under supervision, focusing on long-term condition monitoring, prevention, screening, immunisation and patient education.

The ideal candidate will have at least two years’ experience in primary care, a practice nurse qualification, and experience in chronic disease management and cervical cytology.

Qualifications

• Certified practice nurse with at least two years' experience in primary care.
• Experience with chronic disease management and immunisation programmes.
• Experience in cervical cytology is desirable.

Responsibilities

• Promote health and well-being and prevent adverse effects on health.
• Implement and evaluate treatment plans for long-term conditions.
• Identify patients at risk and manage care plans accordingly.
• Support emergency care and patient adherence to treatments.
• Deliver vaccination and immunisation programmes for adults and children.
