Job summary

NHS Jobs in the United Kingdom is seeking an experienced Practice Nurse with an interest in chronic disease management to join our primary care team on a part-time basis.

The role requires NMC registration, strong IT and communication skills, and the ability to conduct annual reviews, vaccinations, injections, wound care and chronic disease monitoring in line with national guidance.

Qualifications

• Must be a NMC Registered Nurse with primary care experience.
• Strong IT and communication skills with close attention to detail.
• Experience in chronic disease management is desirable.

Responsibilities

• Cervical cytology and venepuncture within standard practice.
• Childhood immunisations and basic travel vaccines.
• ECGs, annual reviews and chronic disease monitoring.
• Administer vaccinations per PGDs and standard guidelines.
• Wound care, suturing and general injections.
• Diabetes management, BP checks and NHS health checks.
• COPD/Asthma management and contraception care.
• Assist with implants/coil procedures and EMIS knowledge advantageous.

Job description

We currently have an opening for an experienced Practice Nurse with an interest in chronic disease management to join our team on a part time basis.

Qualified Nursing applicants must also be able to demonstrate good I.T. and communication skills, with a close attention to detail.

• Cervical cytology
• Venepuncture
• Childhood immunisations
• Electrocardiograms Conduct annual reviews and monitoring for patients with chronic diseases.
• Administer vaccinations and immunisations in accordance with national guidance and Patient Group Directions (PGDs).
• General Injections
• Basic Travel advice and vaccinations
• Wound care and suture removal
• Diabetes management ECGs and blood pressure checks NHS health checks and New patient health checks
• COPD management Asthma management Contraception Assist with implants and coil procedures Wound care QOF and Enhanced Services Knowledge of EMIS would be an advantage.

The successful candidate must be a NMC Registered Nurse with experience of working within Primary Care.
