Job summary

Symphony North (Burnham, Berrow and Highbridge Medical Centres) is hiring a Treatment Room Nurse or Practice Nurse with chronic disease qualifications on a 12‑month fixed-term basis, 24 hours per week across three sites.

You will be part of a Nursing Team supporting preventive care, monitoring long-term conditions and delivering clinical assessments with safe decision-making across practice and home settings.

Qualifications

• Registered nurse with current UK registration.
• Willingness to learn and acquire new skills.
• Experience in General Practice and long-term condition care is advantageous.
• Experience using a GP clinical system (EMIS preferred).
• Post-registration study in topics allied to general practice desirable.

Responsibilities

• Assist the practice team in delivering effective management of patient needs.
• Manage nursing care of patients with chronic disease using recall systems and input data for contracts.
• Contribute to policies, protocols and guidelines within the practice.
• Support new initiatives for chronic disease management and integrated services.
• Provide safe, clinical decision-making and expert care to patients.
