Job summary

Walderslade Surgery in the United Kingdom is seeking an experienced Practice Nurse to work 24 hours per week across Wednesday, Thursday and Friday. The role covers all aspects of Practice Nursing with a focus on chronic disease management.

You will join a friendly team delivering patient-centred care, with opportunities in immunisations, screenings and clinical governance, supported by EMIS Web in a NHS context.

Qualifications

• Experience of working in a primary care environment.
• Experience as a practice nurse or community nurse.
• Experience in chronic disease management (e.g., COPD, asthma).
• Registration as a Nurse with the NMC (NMC registration).
• Postgraduate diploma or degree in Primary Care is desirable.

Responsibilities

• Provide care as part of the primary healthcare team.
• Manage chronic diseases (e.g., COPD, asthma) and related monitoring.
• Carry out immunisations, cytology, swabs and routine nursing procedures.
• Attend Practice meetings and maintain accurate patient records.
• Follow infection control procedures and support GPs where needed.
