Whitstable Medical Practice is seeking a motivated Practice Nurse with experience in Chronic Disease Management to join our multidisciplinary team across Estuary View, Chestfield and Whitstable Medical Centre.

Qualifications

• Competency in basic nursing duties required for the role.
• Chronic Disease Management experience is essential.
• Excellent assessment and clinical decision-making skills.

Responsibilities

• Undertaking comprehensive clinical assessments and developing management plans.
• Monitoring disease control and patient outcomes.
• Supporting patients to understand and manage long-term conditions.
• Recognising when referral to GPs or specialists is required and arranging referrals.
• Maintaining accurate clinical records and safeguarding procedures.
• Collaborating with the multidisciplinary team across three sites.
