The Dale in Wolverhampton is seeking an experienced Practice Nurse to join our friendly team. The role involves delivering primary care services, managing chronic conditions and supporting vaccination programmes.

Responsibilities

• Manage chronic diseases and long-term conditions including diabetes and hypertension
• Deliver cervical screening, vaccines and immunisations, including seasonal flu clinics
• Provide wound care and clinical procedures such as ECGs
• Coordinate travel health services and provide contraception within scope of practice
• Maintain accurate clinical records and ensure SNOMED CT codes are used effectively
• Support MDT working, safeguarding and escalation when needed
• Undertake other general practice nursing duties as required
