Job summary

The Wyre Forest Health Partnership is seeking a motivated Practice Nurse for Stourport Medical Centre to deliver nursing assessments and high-quality care in line with practice policies. The role focuses on chronic disease management, mentoring HCAs, and supporting a skilled nursing team within the partnership.

Ideal candidate is clinically confident, communicates well, and is passionate about delivering excellent primary care services; vaccine administration and wound care are key duties.

Qualifications

• Registered first level nurse with clinical supervision training and experience.
• Knowledge of needs of patients with long-term conditions and health promotion.
• Awareness of clinical governance, local and national health policy, and public health issues.
• INR trained; experience with protocols and quality initiatives.

Responsibilities

• Undertake wound care, management and dressing techniques including leg ulcer management and compression bandaging.
• Administer adult and paediatric injections under Patient Group Direction or prescribing protocol.
• Provide adult and childhood immunisation services following PDs or training.
• Apply infection control measures for specimen handling, waste disposal and sharps injuries; manage emergencies.
