Job summary

St James Medical Centre in Rawtenstall is seeking an experienced part-time practice nurse (15 hours/week over 2 days) to join our friendly team. The post involves managing chronic disease, running vaccine clinics and cytology testing, with emphasis on patient care and quality targets.

The role includes health promotion, immunisations, infection control and mentoring junior staff within a supportive training practice environment.

Qualifications

• Experience of managing Chronic Disease including Respiratory, Heart and Diabetes.
• Experience of running vaccine clinics including childhood vaccines and seasonal vaccines.
• Experienced in taking cytology tests.
• Meeting QOF targets.
• Experience of supporting junior team members.

Responsibilities

• Assess, plan, provide and evaluate nursing care.
• Provide access to GP/PAs.
• Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition.
• Prioritise health problems and intervene appropriately to assist in emergencies.
• Check emergency equipment and records regularly and ensure they are up to date.
• Support patients to adopt health promotion strategies and self-care.
• Maintain health promotion materials in practice and waiting room.
• Undertake health screening, surveillance, and immunisation.
• Maintain patient confidentiality and accurate record keeping.
• Attend and participate in practice team and clinical meetings.
• Vaccination and immunisations with safe vaccine storage and cold chain policy.
• Infection control support and policy adherence.
• Perform venepuncture.
• Mentor and assess junior team members.
