Job summary

NHS Jobs is seeking a proactive team member to support the Neighborhood Health Implementation Programme (NNHIP) pilot work across the PCN. You will collaborate with clinicians and staff to help deliver pilot activities and strengthen patient services within the network.

You will assist the PCN multi-disciplinary team (MDT) to enable high quality patient care, support the PCN Patient Care Projects and Services, and help develop links with our patient community, including the Prescribing Hub.

Responsibilities

• Support the Neighborhood Health Implementation Programme (NNHIP) pilot work across the PCN.
• Support the work of the PCN multi-disciplinary team (MDT) to enable them to provide high quality patient care.
• Support the PCN Patient Care Projects and Services.
• Support the PCN to develop and maintain links with our patient community :
• Support the PCN Prescribing Hub
