Job summary

Connexus PCN in South Bristol is recruiting a Care Coordinator/Admin Assistant to support the Neighborhood Health Implementation Programme (NNHIP) pilot and the Prescription Hub across five practices. You will work with the multidisciplinary clinical team to ensure patients, especially those with complex needs, can access timely services.
