Job summary

Sutton Primary Care Networks is seeking a Proactive Anticipatory Care Neighbourhood Care Coordinator to support delivery of personalised, proactive, and holistic care within a defined PCN caseload. You will work within a multidisciplinary team, coordinating across health, social care, and community services to improve outcomes.

You will undertake home visits, prepare for MDT meetings, and maintain clear notes and actions. A full UK driving licence and access to a car are essential for this role.

Qualifications

• Full, clean driving licence and access to a car for home visits.
• Experience in primary care or community health.
• Strong communication and documentation skills.

Responsibilities

• Support delivery of personalised, proactive, and holistic care within the PCN as part of an MDT.
• Manage a defined caseload of patients and coordinate care across health and social care.
• Prepare for MDT meetings; ensure the right professionals are involved in advance.
• Undertake home visits for assessment and delivery of care.
• Maintain high quality notes and document agreed actions from MDT meetings.
• Signpost patients to appropriate health and wellbeing services to meet care needs.
• Collaborate with community teams, families, carers, and external partners to support coordinated care delivery.
