Job summary

GP Health Connect Limited in Runcorn is seeking a Care Coordinator to support the new Neighbourhood MDT, connecting medical and local community teams to deliver integrated care.

You will coordinate MDT meetings, track patient pathways, maintain records and provide admin and information management support to the multidisciplinary team, helping to improve patient care and organisational efficiency.

Qualifications

• Previous administrative experience, preferably in healthcare or similar setting.
• Experience with data, electronic records and information systems.
• Experience in a multidisciplinary team is desirable.
• Excellent organisation and time-management with ability to manage competing priorities.
• Strong written and verbal communication.
• High level of attention to detail and accuracy.
• Understanding of confidentiality, information governance and data protection.

Responsibilities

• Coordinate regular MDT meetings, including arranging dates, preparing agendas and circulating relevant patient lists and information.
• Attend MDT meetings and accurately record discussions, decisions, outcomes and actions.
• Maintain an effective system for tracking actions arising from MDT meetings and ensure follow-up within agreed timescales.
• Ensure that relevant clinical and administrative information is available to support MDT discussions.
• Act as a key administrative point of contact for MDT-related queries.
