Northampton General Hospital NHS Trust is seeking a Clinical Patient Safety Reviewer to systematically review patient safety incidents, lead after-action reviews, and identify themes across incidents. The role supports governance by presenting findings to the Incident Review Group and ensures timely review of moderate/harm incidents.

The post holder collaborates with the Patient Safety Team to develop thematic reports, contribute to action plans, and promote a culture of learning and safety

Qualifications

  • Educated to degree level or equivalent.
  • Registered healthcare professional with current registration.
  • Evidence of CPD to postgraduate level or equivalent.
  • Experience in healthcare setting and patient safety.
  • Knowledge of PSIRF and AAR methodologies.
  • Experience with incident investigations and data analysis.

Responsabilités

  • Undertake patient safety incident reviews using multiple methods (AARs, Clinical Reviews, Structured Judgement Reviews).
  • Ensure reporting requirements and timescales for incidents are met.
  • Engage staff in clinical areas to support rapid reviews.
  • Coordinate and facilitate AARs in line with PSIRF.
  • Document outcomes and recommendations and ensure follow-up.
  • Support duty of candour and thematic analysis of low/no harm incidents.
