Kingston Hospital is seeking an Assistant Patient Pathway Co-Coordinator (Band 3) to support the patient journey from referral to discharge. The role is based at Kingston Hospital and involves administrative management within a multidisciplinary team.

You will log referrals, schedule appointments, manage outpatient and theatre capacity, and ensure timely pathways with SOP-driven processes. The post emphasizes patient and clinician satisfaction and flexible NHS working arrangements.

Formación

• Customer service experience.
• Admin experience.
• Ophthalmology experience.
• NHS or equivalent health care experience.

Responsabilidades

• Process and log all referrals in line with Trust Access Policy and specialty SOP.
• To use the 18weeks referral to treatment (RTT) to manage all outpatient and elective patient journeys.
• Schedule new and follow up appointments with the most appropriate clinician, ensuring capacity is proactively and efficiently used.
• Schedule elective admissions, contacting patients offering them a choice of admission dates and agree with them the date of admission.
• Manage and monitor outpatient and theatre scheduling making best use of capacity.
