Job summary

Oxford Health is seeking a Patient Flow Coordinator to support safe, timely and person-centred discharges across our community hospitals, including Abingdon Community Hospital. You will ensure effective communication with patients, families, and multi-disciplinary teams to minimise length of stay and readmissions.

Qualifications

• Educated to NVQ Level 3, BTEC, or equivalent qualification/experience in health or social care
• Proven ability to communicate clearly with patients, families and professionals
• Experience working as part of a multidisciplinary team and coordinating care

Responsibilities

• Planning and Coordinating Hospital discharges
• Effective communication with patients and families to involve them in preparing for discharge from hospital
• Working closely with colleagues in other organisations to arrange appropriate support for patients when they leave hospital
• Maintaining clear and accurate documentation
• Coordinating and contributing to daily board rounds and weekly MDT meetings
• Contribute to the induction and training of new staff, including shadowing and mentoring opportunities relevant to the discharge process
• Provide weekend cover as needed and cover other Community Hospitals when required
