Job summary

Oxford University Hospitals NHS Foundation Trust is seeking a discharge co-ordinator on the Complex Medicine Ward at John Radcliffe Hospital. The role focuses on facilitating timely discharges from hospital and hub beds, with emphasis on complex multi-disciplinary assessments and care planning.

You will work closely with nursing, medical, therapy and social care staff, patients and families to manage pathways and ensure safe, efficient discharge decisions under shifting demands.

An opportunity has arisen to work as a discharge co-ordinator on Complex Medicine Ward the John Radcliffe Hospital.

The post holder aims to facilitate effective, efficient and timely discharge of patients from hospital and 'hub' beds. Particular emphasis is placed upon complex multi-disciplinary assessments and case conferences including patients with complex health and social care, palliative care or rehabilitationneeds.

Main duties of the job

The post holder aims to facilitate effective, efficient and timely discharge of patients from hospital and nursing home beds. Particular emphasis is placed upon complex multi-disciplinary assessments and case conferences including patients with complex health and social care, palliative care or rehabilitation needs.

The role will involve close working with all members of the multi-disciplinary team (nursing, medical, therapy, social work and care home staff) and patients/relatives/carers, ensuring safe discharge of patients to the most appropriate place. The successful candidate will play a vital role in managing patient pathways.

We are looking for pro-active, highly motivated individuals to work autonomously or as part of a team. The applicant will be expected to be enthusiastic, organised with excellent communication and IT skills.

They should be able to use their own judgement to resolve complex issues. They should be able to work well under pressure and be able to prioritize their own workload under constantly changing demands.

The post holder is expected to work alternate shifts including weekends.
